Responsibilities
+
Employer:
+
+
Evaluate the number and types of hazards expected based on past experience and general knowledge to plan and develop Emergency Plan specifics.
+
Provide training to all employees for their roles in all emergency plans
+
Annually conduct necessary drills to exercise the emergency response plans
+
Conduct all other actions required in this planning guide to implement, develop and maintain an effective Emergency Plan.
+
Assist the employer in the development and execution of emergency plans.
+
Perform assigned tasks under the emergency plan.
+
+
+Code Red Reporting Procedures:
+ The method of Emergency reporting used at J.V. Manufacturing, Inc. is an intercom system. The intercom can be accessed from any phone throughout the facility. The individual who recognizes the emergency will pick up the nearest phone and report the emergency immediately.
+ If a fire is identified, then employee must use his/her discretion as to whether it can be extinguished. If the fire is too large, then the coding system will be used. The following are the instructions for reporting a CODE RED:
+
Pick up phone, wait for dial tone. Dial extension 634. Wait for the beep, then announce CODE RED and the building NUMBER/NAME. Hang up. phone. (Instructions are located by all phones throughout the shop.) Below is a list of the building #’s and names:
+

Receptionist will dial 911 and notify operator of the type of emergency and location. The HR department will be the back up incase receptionist is not available.

Code Red Evacuation/Employee Accountability Procedures:
+
+
When a code red is announced over the loud speaker, all employees will need to evacuate their work areas immediately in a safe and calm manner. This means no running or joking.
+ All employees are trained on their area’s evacuation exits, routes, and meeting places. Attached is the corresponding map with the evacuation routes. There are also evacuation maps throughout the facility.
+ All employees must meet with their supervisor and fellow employees at the designated evacuation destination.
+ When you reach your destination, please remain in a single file line with your department. Please DO NOT mix in with other departments.
+ If you are in another building when the code is sounded, it is important that you meet up with your department at the evacuation destination.
+ This will help with employee accountability.
+
It is the area supervisor’s responsibility to make sure that everyone in his/her department is accounted for at the evacuation destination.
+If the supervisor is absent, it is the responsibility of the safety committee member for that department to account for all employees.
+The plant superintendents will contact each line supervisor/safety committee member to verify status of employees.
+If an employee is not accounted for, the emergency officials on site will be notified immediately for a rescue attempt.
+
No one is allowed to leave the evacuation area for any reason until the emergency officials on site give the “all clear.”
+
+
+Code Yellow Reporting Procedures:
+ The Safety Coordinator and/or the Plant Superintendent will watch the weather service bulletins and announce a CODE YELLOW if there is a chance of a tornado. If there is enough notice from the National Weather Service, the facility will be placed on an alert status prior to announcing the code.
+
+
When the CODE YELLOW is announced over the intercom system, all employees must proceed to their departments assigned evacuation destinations. They are as follows:
+
Building #’s 1,2,3,3a will evacuate into the Engineering/Sales Department
+
Building #4 will evacuate into the Install Department
+
Building #5 and Paint Room will evacuate into the electric room.
+
Engineering Department and Install Depart will stay in their area.
+
Service Department, Accounting, and Executives will evacuate to the conference room and hallways bordering it.
+
It is the supervisor’s responsibility to make sure that everyone in his/her department is accounted for at the evacuation destination. If the supervisor is absent, it is the responsibility of the safety committee member for that department to account for all employees. The plant superintendents will contact each line supervisor/safety committee member to verify status of employees.
+
No one is allowed to leave the evacuation area for any reason until the “all clear” signal is given by the either the Safety Coordinator or the Plant Superintendents.

Training:
+
+
All employees of J.V. Manufacturing, Inc. will be trained on the Emergency Action Plan when:
+
Associate's responsibilities under the plan change
+
Whenever the plan is changed.
+
For all new Employees as part of the New Employee Safety Training.
+
Periodic drills will be performed to assure that all employees know what to do incase of an emergency.
+Persons with specific duties require additional refresher training throughout the year.
+Local Emergency Services will be contacted and invited to train in conjunction with company drills.
+The written plan is maintained in the workplace and made available for employee review.
+

Machine Guarding
+
Definitions:
+
Guards - Barriers that prevent employees from contact with moving portions or parts of exposed machinery or equipment which could cause physical harm to the employees.
+
Enclosures - Mounted physical barriers which prevent access to moving parts of machinery or equipment.
+
Point-of-Operation - The area on the machine where work is being done and the material is positioned for processing.
+
Power Transmission - Any mechanical parts which transmit energy and motion from a power source to the point-of-operation.
+
Shear Points - The reciprocal (back and forth) movement of a mechanical part past a fixed point on a machine.
+
Shearing Action - The danger of this type of action lies at the point of operation where materials are actually inserted, maintained and withdrawn.
+Guarding is accomplished through fixed barriers, interlocks, remote control placement, feeding or ejection.
+
Nip Points - In-Running Machine or equipment parts, which rotate towards each other, or where one part rotates toward a stationery object.
+
Rotating Motions - an exposed mechanism are dangerous unless guarded. Even a smooth, slowly rotating shaft or coupling can grasp clothing or hair upon contact with the skin and force an arm or hand into a dangerous position. Affixed or hinged guard enclosure protects against this exposure.
+
Reciprocating - Reciprocating motions are produced by the back and fourth movements of certain machine or equipment parts. This motion is hazardous, when exposed, offering pinch or shear points to an Employee. A fixed enclosure such as a barrier guard is an effective method against this exposure.
+
Transverse Motions - Transverse motions are hazardous due to straight line action and in-running nip points. Pinch and shear points also are created with exposed machinery and equipment parts operating between a fixed or other moving object. A fixed or hinged guard enclosure provides protection against this exposure.
+
Cutting Actions - Cutting action results when rotating, reciprocating, or transverse motion is imparted to a tool so that material being removed is in the form of chips. Exposed points of operation must be guarded to protect the operator from contact with cutting hazards, being caught between the operating parts and from flying particles and sparks.
+
+
Machine Guarding Requirements:
+
+
Guards shall be affixed to the machine where possible and secured.
+
A guard shall not offer an accident hazard in itself.
+
The point-of-operation of machines whose operation exposes an employee to injury shall be guarded.
+
Revolving drums, barrels and containers shall be guarded by an enclosure which is interlocked with the drive mechanism.
+
When periphery of fan blades are less than 7 feet above the floor or working level the blades shall be guarded with a guard having openings no larger than 1/2 inch.
+
Machines designed for a fixed location shall be securely anchored to prevent walking or moving. For example; Drill Presses, Bench Grinders, etc. If you notice one not anchored, please notify your supervisor.
+

Hand & Power Tools
+
The proper use of tools makes tasks easier. However, the same tools that assist us can, if improperly used or maintained, create significant hazards in our work areas.
+
+
Powered Abrasive Wheel Tools:
+
+
Powered abrasive grinding, cutting, polishing, and wire buffing wheels create special safety problems because they may throw off flying fragments.
+
Before an abrasive wheel is mounted, it should be inspected closely to determine if it is free from cracks or other defects. To test, wheels should be tapped gently with a light non-metallic instrument.
+If they sound cracked or dead, they could fly apart in operation and must not be used. A sound and undamaged wheel will give a clear metallic tone or “ring.”
+
To prevent from cracking the user should be sure it fits freely on the spindle. The spindle nut must be tightened enough to hold the wheel in place, without distorting the flange. Follow the manufacturer’s recommendations.
+
Due to the possibility of a wheel exploding, during start up, the employee should never stand directly in front of bench grinders until the wheel accelerates to full operating speed.
+
Portable grinding tools need to be equipped with safety guards to protect workers from the rotating wheel and flying fragments.
+
+
+
Powered Grinder Safety Precautions:
+
+
Always use eye protection (safety glasses and a face shield.)
+
Turn off the power when not in use.
+
Never clamp a hand held grinder in a vise.
+
+
+Pneumatic Tools are powered by compressed air. Examples are chippers, drills, grinders, hammers, and sanders. There are several dangers encountered in the use of pneumatic tools.
+
+
Pneumatic Tools Safety Precautions:
+
+
Always be sure the air pressure in the lines being used is compatible with the type of tool being used. Most tools have a recommended operating level. If you have any concerns, contact maintenance or your supervisor.
+
When using pneumatic tools, employees are to check to see that they are fastened securely to the hose to prevent them from becoming disconnected.
+
Compressed air tools of any type should never be pointed towards anyone.
+
If compressed air is used for cleaning purposes, the nozzle must be regulated to a pressure below 30 psi.
+
Always use safety glasses and face protection when operating pneumatic tools.
+
+
+Guarding - Hazardous moving parts of a power tool need to be safeguarded. Guards protect the operator and others from the following:
